Introduction: As the life expectancy of people is increasing , old age population also and trochanteric fractures with osteoporosis are getting commoner producing challenge regaining there pre operative activity status for which a cemented bipolar arthroplasty was performed as an alternative to osteosynthesis by Dynamic screws . Harris hip score assesed at various intervals within year.
 Materials Methods: This prospective study including 25 cases unstable intertrochantric fracture (classified according Evans classification) treated prosthesis assessed
 Result: The mean Score 78.86 end one Discussion: satisfactory level post op improved over first six weeks later it stabilzes till year
 Conclusion: authors believe hemiarthroplasty useful in osterporotic ustable fractures.
